Providing companion and assistance with light household tasks to individuals and family to improve and maintain the quality of life in their own home setting. Providing aide and also training the clients with their activities of daily living, such as eating, bathing, dressing, personal care to everyone, and medication administration or preparation and management. We also provide care to clients who may be bed or wheelchair bound with gastric tube feedings, tracheostomy care, and clients who may require frequent turning and have other needs. These services are integrated to educate/teach homemaking skills, to enable individuals to remain in their own homes and community and performs their related duties and responsibilities as required.
Personal Support / Companion Care

Providing life skills development that includes non-medical care, supervision, and social interaction activities such as (body care, playing board games, puzzle, bowling, reading a book and movies etc.), for Recipients age 21 years and older. These activities provide access to the recipient’s community.
Life Skills Development

Providing training and assistance in a variety of activities to recipients limited to adult age 18 years or older who live in their own homes or apartments. Also providing assistance with locating appropriate housing or with the improvement of skills related to their activities of daily living such as personal hygiene and grooming, household chores, meal preparation, shopping, personal finances and the social and adaptive skills necessary to enable recipients to reside on their own.
Supportive Living Coaching

Providing continuous or intermittent nursing services primarily in the comfort of your home by a Licensed Practical Nurse (LPN) and or Registered Nurse (RN), Home Health Aid and or Certified Nursing Assistance (CNA).
Private Duty Nursing

Aiding with temporary relief to a primary caregiver from the continuous support and care of children, adult, and elderly with developmental disabilities. Assisting in the maintaining of the client’s own home and property as a clean, sanitary, and safe environment. This service is generally used or utilized due to a brief planned or emergency absence or when primary care giver is available but is currently experiencing a temporary physical limitation due to own medical condition such as undergoing a surgery and is unable to care for or supervise the client.
Respite Services

Provides continuous or intermittent nursing services primarily in licensed residential facility by Licensed Practical Nurse (LPN) or Registered Nurse (RN), Certified Home Health Aid or Certified Nursing Assistance (CNA).
Residential Nursing

Provides continuous or intermittent nursing services in the home, in the community, and other waiver service sites, such as adult day training program. Providing part-time or intermittent nursing care visits daily by a Licensed Practical Nurse (LPN) and or Registered Nurse (RN), Certified Home Health Aid and or Certified Nursing Assistance (CNA).
Skilled Nursing

Provides to Doctors’ Appointments, Banking, Shopping, Entertainment, Religious Services.
Transportation

Supportive Housing for adults provides housing and support services for persons with or without Disabilities.
Group Home / Supportive Housing
